Earning a Doctorate, Supriyanto Promotes Innovation in Quality and Affordable Healthcare Services

SURABAYA, KOMPAS – Hospital management innovations need to deliver high-quality, affordable services through the concept of a hospital without walls. This innovation, supported by the use of information technology, is considered relevant for implementation in all hospitals in Indonesia.

The implementation is believed to enhance the achievement of key health indicators in Indonesia. Currently, the indicators still show significant disparities, particularly in maternal and infant mortality, heart attacks, and traffic accident casualties.

The idea was presented by the President Director of Cipto Mangunkusumo Hospital (RSCM) Supriyanto Dharmoredjo in his dissertation, which was showcased during the open final examination of the second stage doctoral program in Public Health at Airlangga University Surabaya, Friday (23/1/2026).

The open examination led by the Rector of Unair, Professor Madyan, was attended in person by the Minister of Health Budi Gunadi Sadikin and the Deputy Minister of Higher Education, Science, and Technology Fauzan. Also present online were the Coordinating Minister for Human Development and Culture Supratikno and the Deputy Governor of East Java Emil Elestianto Dardak.

Hospital management based on the concept of hospital without walls (HWW) is not a new phenomenon in the world. In Indonesia, this concept has been successfully implemented at the dr Iskak Tulungagung Regional Hospital, which he led for 10 years, in Tulungagung Regency, East Java. The formulation of this concept began in 2012 and has resulted in international recognition.

In 2019, the public hospital was awarded as the best hospital in the world for its public service (corporate social responsibility) by the International Hospital Federation. RSUD dr Iskak received the Gold Award after surpassing several hospitals worldwide, such as Aster DM Healthcare and Dubai Authority from the United Arab Emirates, Auna from Peru, KPJ Pasir Gudang Specialist Hospital from Malaysia, and Manila Doctors Hospital from the Philippines.

Supriyanto stated that Tulungagung is an example of a region with geographical and demographic complexities that demand an integrated emergency system. The HWW concept is highly relevant as a framework for transforming emergency services that implement clinical and managerial functions of hospitals within a single ecosystem of continuous services.

This concept emphasizes the importance of strengthening efforts in pre-health service facilities that are integrated and coordinated, enabling a quick and effective response in various emergency situations.

According to him, the issue of emergency care is not solely clinical but also a managerial and systemic problem involving many actors across organizations. Dependence on hospital-centered services and fragmentation of coordination among stakeholders has resulted in the emergency care service chain not functioning optimally.

Amid the increasingly complex services, the emergency response system is required to adapt quickly and across sectors. This is because reliance on the physical structure of hospitals and conventional bureaucracy creates critical gaps in emergency management, which directly impacts the increase in morbidity and mortality.

Therefore, emergency services are needed that can expand the reach of services beyond the physical boundaries of hospitals and ensure continuity of care from pre-hospital, intra-hospital, inter-hospital, to post-hospital.

After being implemented at Dr. Iskak Tulungagung Regional General Hospital, hospital services using the HWW concept, enhanced by information technology within the public safety center (PSC) platform, are now being implemented at Cipto Mangunkusumo Hospital (RSCM). The concept has also been expanded to include not only emergency services but also non-emergency services.

Currently, RSCM has successfully improved the quality of public services, reduced healthcare costs, and addressed the financial crisis that the hospital had experienced for years. Its human resource management has also become more efficient and effective. The wage payment system is performance-based.

On that occasion, Minister of Health Budi Gunadi Sadikin inquired about the opportunities to replicate hospital services with the HWW concept, reinforced by information technology within the public safety center (PSC) platform, across 514 regional hospitals in Indonesia. The aim is to ensure that all secondary services can treat patients quickly, easily, and affordably.

In this regard, according to Supriyanto, a strong commitment from policymakers is necessary, supported by legislation. At a minimum, support is required from four ministries, namely the Ministry of Health, the Ministry of PMK, the Ministry of Finance, and the Ministry of Diktiristek.

In addition, support from the legislature, in this case the House of Representatives (DPR), is necessary, particularly concerning the drafting of regulations as a legal umbrella and the allocation of budget from the Ministry of Finance.

“I have spoken with all the directors of regional hospitals, military hospitals, and police hospitals. The key lies in transformational leadership (a leadership style that inspires change through vision, motivation, and empowerment),” said Supriyanto.
